How do I replace a Canadian passport that is lost, stolen or damaged?

If your Canadian passport is lost or stolen, report it as soon as possible: by phone at 1-800-567-6868 from Canada or the United States, or with the nearest Government of Canada office if you are abroad. If the passport is still valid, you submit a replacement application with a declaration (form PPTC203) and pay the usual fees, plus an additional fee for a lost or stolen passport ($45 according to the page we read). An expired passport is replaced without a special form, through a renewal or a new application. A review or investigation into what happened can delay the replacement.
