How do I replace a lost or stolen health insurance card?

- Online cost
- $15, payable by credit card (Visa or MasterCard).
- Cost by mail
- $25 by cheque, bank draft or money order, when you return the form.
- Free replacement
- Replacement is free for people aged 65 or older and for recipients of a financial assistance program with a claim booklet.

- Within 2 weeks of receiving your complete application; this can stretch to 3 weeks when the RAMQ receives many applications.
- Your new card
- Same photo, signature and expiry date as the card being replaced.
