The death certificate and the copy of the death act are the official proofs of death issued by the Directeur de l'état civil. The death must first be declared and registered in the Québec civil status register; only then can a request for a certificate or copy of act be processed. - Times exclude the registration time for a new event in the register (if it is recent) and the delivery time. Registration of a death is listed at 20 business days if the documents are complete and compliant.
- Identity
- By mail: two photocopies of documents issued by two distinct organizations (a valid identity document with photo and a valid proof of address). At the counter: the originals.
Steps
- Make sure the death has been declared to the Directeur de l'état civil. In general, this declaration is made through the funeral services company that takes charge of the body of the deceased person.
- Wait for the death to be registered in the civil status register: a request for a certificate or copy of act can only be processed after registration.
- Choose the right document: the certificate presents an extract of the information in the act, while the copy of the act is a full reproduction. Check what the organization asking for it requires.
- Check that you can apply: you must be named in the act or justify your request. If you are the spouse, civil union partner, child, brother or sister of the deceased person, you do not have to provide an official document justifying your request.
- Choose how to apply: online with DE Clic!, by mail (form for death), or at a Services Québec office that offers the service. The funeral services company representative can also make the request online for you or give you a paper form.
- Choose the processing time: normal processing of 10 business days or accelerated processing of 3 business days (the price changes), then pay the fees.
