Steps
- Check that you meet the criteria: have a health insurance card, be 14 years of age or over, and not be under tutorship or curatorship or in a situation where another person represents you. The health information of a child under 14 is included in the parents' record.
- Go to the Québec Health Booklet page on Québec.ca and choose "Access Carnet santé Québec", or go through the carnetsante.gouv.qc.ca portal ("Connexion" and "Nouvel utilisateur" buttons).
- If you already have a Government Authentication Service account, log in with it. Otherwise, create one: choose a user name, enter your email address, create a secure password, then validate the account with the unique six-digit code sent to your email. You need a unique email address, linked to only one account.
- Prove your identity (step 2 of the account creation) with your health insurance number, your social insurance number, your notice of assessment number from Revenu Québec and the reference number of your valid driver's licence or valid health insurance card.
- If you do not have your notice of assessment number or either of the two reference numbers, you can request an identification secret by mail. Allow up to 15 days to receive it.
- Once logged in, look at the available sections: medications, test results, medical imaging reports, fee-for-service medical services, and the list of workers who have accessed your health information.
