- Before confirming that you have no access to dental coverage, check the code on your T4 slip (box 45) or T4A slip (box 015): the number 1 means your employer or pension plan administrator reports that you have no access to it. Also check the slips of your spouse or common-law partner.
- Gather the information requested for each person included in the application: social insurance number (if available for children), date of birth, full name, home address and mailing address, and any dental coverage from government social programs, if applicable.
- Apply online: sign in to (or create) your My Service Canada Account, go to the "Canadian Dental Care Plan" section of the dashboard and click "Apply for the Canadian Dental Care Plan". If you cannot use My Service Canada Account, there is an option to apply through Canada.ca, or by phone at 1-833-537-4342 (TTY: 1-833-677-6262).
- Your spouse or common-law partner must submit their own application to be covered: the information you give about them is only used to calculate your adjusted family net income.
- Wait for the confirmation letter or email (depending on your preferred way of communicating). It shows your member number, the start date of your coverage and your co-payment level. Do not book an appointment before the start date.
- Book an appointment with an oral health care provider and confirm at every appointment that your coverage is active. You should not have to pay the full cost up front, but you may have to pay your co-payment and possible extra fees directly to the provider.
