  1. Choose how to proceed: online, in person or by mail. Requests cannot be made by phone.
  2. Online (people aged 14 and over): go to the Votre Santé portal. At the bottom of the page, in the “Consulter vos renseignements de santé” (View your health information) section, sign in with your Service d’authentification gouvernementale account. Click “Vaccination”, then the download icon to keep a copy.
  3. In person: contact the health and social services institution in your region to find out the procedure, then bring valid ID, such as a health insurance card or a birth certificate. This option also lets you make a request for a person under 14; you must then also bring valid ID for the parent or guardian.
  4. By mail: fill out and sign the request form for access to vaccination information, attach a photocopy of valid ID, then send your request to the person responsible for your region (the list is on Québec.ca). If you live outside Québec, the address of the ministère de la Santé et des Services sociaux is given on the official page.
  5. For mail requests, allow a response time of about 20 calendar days, which can vary with the volume of requests. No acknowledgment of receipt is sent.
  6. If some information in your record is inaccurate or incomplete, contact the health and social services institution in your region to find out the procedure for an update or correction.
  7. Also keep your paper vaccination booklet and your children’s, and keep them up to date: the Vaccination Registry does not replace these documents.
